PLOT
FBI agent Jake Malloy (Sylvester Stallone) has been pursuing a serial killer who specializes in killing police officers without much luck. Then Malloy's old police partner from his Seattle Police Department days, was killed by the cop killer. Afterwards, the murderer calls Malloy at his partner's home phone and learns he is at his house with his girlfriend Mary (Dina Meyer) still inside. The killer tells Malloy that he pursued him four years before on a series of prostitude murders and almost caught him, to which that he holds a special grudge against Malloy and now targets Mary. After the killer hangs up, Malloy rushed back to his house to find Mary hanged to death by the killer. When the killer's hideout was found, Seattle PD and the FBI blockaded the area. Malloy arrives at the scene. Still in shocked by Mary's murder, he pursues the killer and appeared to have committed suicide before Malloy could kill him.
Three months later, Malloy descends into alcoholism over Mary's murder. After Malloy slits his wrists in an unsuccessful suicide attempt, his best friend and supervising officer, Agent Chuck Hendricks (Charles S. Dutton), enrolls Malloy in a rehabilitation program designed exclusively for law enforcement officers. At the isolated area in Wyoming, Malloy sees the rehab center used to be anbandoned US military installation. The program director of the clinic, Dr. John "Doc" Mitchell (Kris Kristofferson), a former cop and recovering alcoholic, estabished the old outpost as a rehab center for cops as him dealing with his own problems. Hendricks decides to stay in Wyoming for awhile to make sure Malloy will be okay throughout the treatment.
There, he meets several other officers who are patients in the clinic, which includes Peter Noah (Robert Patrick), an arrogant and paranoid ex-SWAT officer, Frank Slater (Christopher Fulford), a cynicial, opinionated British police officer, Willie Jones (Courtney B. Vance), a religious homicide detective, Jaworski (Jeffrey Wright), an alcoholic narcotics cop with a bullet scar in his left cheek due to an botched attempt attempt, Lopez (Angela Alvarado), a bad-mouthed LAPD officer and McKenize (Robert Prosky), an elderly member of the Royal Canadian Mounted Police who saw his partner get killed. He also meets several staff members, including Doc's assistant and mechanic Hank (Tom Berenger) and compassionate resident psychiatrist and nurse Jenny Munroe (Polly Walker), the latter who Malloy develops a bond with. Later on, a blizzard forms and seals them in the asylum with no form of communication due to the storm knocking out the lines.
Next day, Jenny finds the body of Connor (Sean Patrick Flanery), one of the troubled patients who apparently killed himself. Jenny suspects that Connor probably didn't killed himself because a patient would've come to her if they we're having suicidal thoughts. The next morning, Hank finds a body of Carl Brandon (Anthony J. Mifsud), another patient, in a holding cell that is used as a room for patients. Brandon seemed to have killed himself, but Malloy checks him out and finds that he didn't killed himself which confirms a psychopath is lurking among them, killing both staff and Malloy's comrades one by one.
Doc, who can't risk having more dead bodies on his watch, decides to lock up all of the surviving patients in their cells so he can look at every patient's case files looking for answers. Jenny informs Doc that Jack Bennett (Stephen Lang), a deranged employee who was a former patient in the clinic, has gone missing along with one snowmobile. While all of the patients are getting locked up, Malloy convinces Jenny to lock her room for her own safety, which she reluctantly does so. While reviewing the case files, Doc hears a commotion and sees his office is ransacked and sees a shadow of someone and follows it outside the building, thinking it was Jack. Then the man with an axe kills Doc. Everyone, but Malloy and Jenny, think Jack is a killer responsible for killing three people. Malloy goes to the safe and hands the surviving cops their sidearms for their own safety against a killer.
Hendricks was going ice-fishing when he finds a body that came out of the frozen lake and finds out from the owner of the nearby fishing shop (Rance Howard) that the victim that Hendricks found was a cop. Realized that Malloy maybe in danger, Hendricks decides to get back to the asylum as quick as possible. He and the owner head for the clinic in a snowcat.
Hank, along with the clinic's cook Manny (Hrothgar Matthews) and helper Gilbert (Alan C. Petersen), volunteer to drive through the blizzard in a pickup with them carrying snow shovels to get outside help. Malloy wants Jenny to leave the clinic as well, but Jenny objects to it, because she has medical expertise and is safer around Malloy and other armed officers. As Malloy investigates the murders, Hank driving the pickup sees something in front of him and veers away from it, but slids off due to ice on the remote road and crashes into a tree at the ravine. Malloy and Jenny heard a car crash outside the building and realized the truck that Hank and other staff members with him crashed. Malloy hands over one of the guns to Jenny for protection and goes investigate the crash. Out in the snow, Malloy finds Manny dead at the road, murdered by the killer after he survived the crash and also finds Jack, whose body was hanging all along in front of the road that caused Hank to crash the pickup. After seeing the wrecked vehicle empty, Malloy gets started by Gilbert coming up behind him. Gilbert, alive, but scared to death, runs away from him. Malloy, realizing there is danger at the asylum, rushes back to the clinic.
McKenize, who is patrolling the old execution chamber, finds the machine is activated and the killer lures him to a trap. The madman electrocutes McKenize to death with the execution chamber, cutting the building's main power and heating system in the process. Malloy forces everyone except Jenny to his cell as he learns one of the cops is a killer impersonating one of the patients. Suspecting the man responsible is a cop killer who killed Mary, Malloy exams Connor's body in the kitchen. Remembering the killer saying "I see you, but you don't see me.", Malloy looks inside Connor's eyelids and finds "I" on one eyelid and "CU" on the other, which means "I See You". That confirms that Mary's killer is alive and is in the outpost. As Malloy and Jenny, who is worried about Malloy's mental state, head back to the cells, Hank appeared and, thinking Malloy might be a killer, knocks Malloy out. He then locks him in Slater's cell and released everyone else.
Malloy, while wrongly locked up, finds a vent and uses a ring he was going to propose Mary with, as a shrewdriver, but as he uses a matches as a light in the cell, Malloy finds out it came from the restaurant he hangs out at in Seattle, which reveals Slater is the killer and he's the only patient who smokes in the clinic. Malloy learned about the rehab center at the restaurant, which Slater learned about at the same time and stalked him there, stealing the identity of the cop he killed and dumped in the frozen lake, which Hendricks later found. Malloy escapes the cell and finds the missing badges above Slater's room, which he collects as trophies. He hands over the prove of that Slater, not Malloy, is the killer with the missing badges to everyone in the patient room. He has Jones and Lopez conduct patrol while while he has Jaworski stay with Jenny in the patient room, much to her chagrin, and heads for the tunnels downstairs.
Meanwhile, Hank, Noah and Slater go to the tunnels to get some logs they can used to light up the fire in the fireplace at the patient room. Without Hank and Noah realizing that Slater is a true killer and knowing that his cover is blown by Malloy, he pretends that they have someone coming for them, which Noah allows them to split up, allowing Slater to easily kill them while they we're alone. As Malloy patrols the tunnels, Slater taunts on the CB radio., and while talking on the radio, lures Malloy to Noah's hung body where the radio which is put on speaker was at. Slater was about to leave the clinic, but hears the worried Jenny, after hearing Malloy's shots and sends out to find him, calling out Malloy's name while outside the snow. Malloy learns that Slater is at the tunnel's outside door and is going to kill Jenny. He rushes to save her.
Outside the installation, Hendricks and the owner find Gilbert, frozen and shaken, but alive and bring him inside the snowcat. They arrived at the rehab center. Hendricks sees Jenny's foot tracks at the front of the outpost and follows it.
Jenny runs to the nearby military shed and hides there with Slater searching for her. Malloy arrives in the shed and tells Jenny to stay inside the shed. Malloy and Slater search for each other outside the shed. Slater seemed to catched Malloy from behind, but is surprised that he caught Hendricks instead and Malloy catches him from behind. Caught off-guard, Slater jumps inside the shed and knocks Jenny out. Malloy and Slater get into a gunfight. Hendricks shoots at Slater from the outside, but gets wounded by him. Malloy shot and grazed Slater's shoulder, but runs out of ammo. The psychopath tries to kill the unconscious Jenny, but Malloy rams himself through the water buckets where Slater was at, preventing him from killing Jenny. During their fight, Slater stabs Malloy's left forearm. Soon, Malloy gets the upper hand and impales Slater to the spiked wheel. After Malloy pulls the knife out of his wounded forearm, the dying Slater tells him that Mary called out his name before he killed her. Enraged, Malloy pulls Slater off the wheel, holds him over his head and said to him "I see you...You see this.". Malloy then throws Slater back to the spiked wheel and kills him instantly.
Wounded by the stab wound and exhausted, Malloy seats down to compose himself. Jenny regains consciousness. The wounded Hendricks gets in the shed and is releived to see his best friend alive. As Jenny takes Malloy and Hendricks back to the outpost for medical attention, Malloy puts the ring he was going to propose Mary with to a tree and leaves it there, indicating he is ready to move on with his life and has feelings for Jenny. As he is walking away from the ring, the glimpse of Mary is seen and then it disapears as the ring is blowing by the howling blizzard winds.
